APPENDIX DClermont County Mental Health and Recovery BoardSeverely Mentally Disabled (SMD) DefinitionThe Clermont County Mental Health and Recovery Board, in concert with the adult mental health provider, has developed a County definition of Severely Mentally Disabled (SMD). The Board is committed to serving the SMD population, and with limited funding and resources has decided that limiting the definition to those with the most severe disability and need would allow the limited resources to be utilized in the most effective and efficient manner. Crisis services are still available at the Clermont Counseling Center for adults regardless of the severity of their mental disorder.To be considered severely mentally disabled and eligible for enrollment in the Clermont County SMD Plan of Services, an individual must meet two (2) of the following three (3) criteria:Must have a DSM-IV-TR diagnosis between the diagnostic codes of 200-299. These diagnoses are considered to be the major mental illnesses. A Diagnostic Assessment including the diagnosis and functional impairments must be completed within the last 10 years for all clients open in the SMD Plan of Services. For new clients referred for services, a thorough Diagnostic Assessment must be completed within the last year;Must have at least two (2) psychiatric hospitalizations, total, or one (1) psychiatric hospitalization with a stay of 12 days or longer within the past 10 years;Within the preceding five (5) years, must have a significant functional impairment secondary to their mental illness in three (3) of seven (7) major life areas. A functional impairment is one that requires a therapeutic intervention in order to obtain/re-obtain or maintain a major life skill in order to become self-sufficient in the community. These functional impairments are identified in the Diagnostic Assessment and are clearly identified as a goal in the client’s Individual Service Plan (ISP). The documented goal can be one that achieves and/or maintains an individuals’ ability to effectively address the functional impairment.
